Our Clinical Lactation Service is a physiotherapy-led programme designed to manage the physiological and mechanical aspects of breastfeeding. By combining clinical manual therapy with Therapeutic Ultrasound, we provide a non-invasive, evidence-based approach to resolving complex breast health issues. This service focuses on optimising the "milk ejection reflex" and maintaining the health of the mammary tissues throughout the lactation journey. In addition to clinical treatment, our programme provides expert education on baby latching, optimal breastfeeding positions, and personalised advice to ensure a comfortable and sustainable nursing experience.

This service is prescribed for anyone experiencing: 

Plugged Ducts (Milk Stasis): Localised pain, redness, or palpable lumps in the breast.

Severe Engorgement: Excessive breast fullness and hardness that prevents milk removal. 

Low Milk Supply (Hypogalactia): Patients needing physiological stimulation to increase production. 

Recurrent Breast Inflammation: For those prone to repeated blockages or inflammatory "clogs." 

Ineffective Latching: Support for mothers whose infants have difficulty maintaining a functional latch due to breast tissue tension. 

Why is the Service Clinically Important? 

Lactation is governed by a delicate balance of hormones (Prolactin and Oxytocin) and mechanical drainage. When milk flow is obstructed, it can lead to severe physical complications. This service is essential for: 

Resolution of Ductal Obstruction: Utilising manual techniques and ultrasound to break down milk stasis and "stones" (calcified milk) that block the ducts. 

Management of Engorgement: Reducing interstitial edema (swelling) that compresses milk ducts and makes latching difficult. 

Optimisation of the Let-Down Reflex: Stimulating the sensory pathways that trigger Oxytocin release, ensuring the contraction of myoepithelial cells for efficient milk expression. 

Prevention of Mastitis: Early clinical intervention to clear blockages and reduce inflammation, preventing the progression to systemic infection. 

Advanced Modality: Therapeutic Ultrasound 

When manual therapy alone is insufficient, we utilise Therapeutic Ultrasound to assist in recovery. 

How it works

Ultrasound waves produce deep-tissue micro-vibrations (thermal and non-thermal effects) that help to liquefy thickened milk and increase localised blood flow.

Clinical Benefit:

It effectively softens "hard spots" or lumps that are too painful or deep for manual massage, significantly reducing treatment time and maternal discomfort.

Education & Advice

Beyond clinical treatment, we provide essential education on baby latching techniques and breastfeeding positions to optimise comfort and milk transfer. Our team offers personalised advice on managing your supply and nipple health, ensuring you have the practical skills and confidence for a pain-free, successful breastfeeding journey.

  • The first session (60 minutes) involves a formal diagnostic review:

    • Breast Tissue Examination: Palpation to locate obstructions and assess for signs of inflammation or infection. 

    • Lactation Mechanics Analysis: Assessment of breastfeeding or pumping techniques to identify mechanical stressors. 

    • Ultrasound Screening: Determining if Therapeutic Ultrasound is indicated based on the depth and density of the obstruction. 

    • Pressure Point Integration: Selection of clinical points (such as Shaoze and Danzhong) to facilitate milk flow and emotional regulation. 

  • Subsequent sessions focus on clinical resolution and education: 

    • Specialized Manual Drainage: Professional massage targeting the axillary lymph nodes to reduce pressure and "clear the path" for milk flow. 

    • Targeted Ultrasound Therapy: Application of ultrasound to specific quadrants of the breast to resolve persistent obstructions. 

    • Lactation Guidance: Practical instruction on breastfeeding techniques, including positioning and achieving a functional latch to ensure efficient feeding and nipple comfort. 

    • Physiological Education: Guidance on the "Demand and Supply" principle and how rest and hydration impact your milk supply signals. 

    • Self-Care Protocol: Training on safe hand-expression and the clinical use of cold compresses to manage inflammation between sessions.
